Information for trail wardens
When you open the files below, the software will advise you that you cannot save a filled Adobe pdf form unless you have a full version of Adobe Acrobat. When you fill in the form, you can only print it or submit it. When you click on the "Submit" button, this is what will happen:
The data in the form is sent to a server which receives the data, creates another Adobe pdf file with the data stored inside. This file is then sent to the specified email recipient as an Adobe pdf attachment. The recipient then saves the email attachment, which is your report.
The fillable form is 1.41 megabytes (mb) in size. If you are accessing the internet via a dial-up connection please be patient while the form downloads. If you have any problems please contact Michael Zinck.
